Several people are involved in the placement of an individual:

  • The individual
  • Their Family or Authorized Representative(s) (AR) or Guardian
  • Support Coordinator (SC)
  • Residential Team Members
  • Day Services Team Members

 

Admission Criteria:

  • Diagnosis of Intellectual Disability
  • Adult
  • Private Pay OR Medicaid Waiver
  • Program Participation

 

A comprehensive screening will occur when interest in HH is communicated.

This screening will include, interviews with the individual, family, AR and/or Guardian, support coordinator and the gathering of several records:

 

Legal information to include: Guardianship (if applicable) OR AR Survey and Rationale (if applicable)

 

Medical information:

  • Diagnosis with information regarding past, current and possible treatment(s)
  • Allergies: Food/Meds/Other
  • Current Medication Orders
  • Physical
  • Immunization Records
  • Psych Evaluation
  • TB Test
  • Communicable Disease Evaluation Results if applicable (HepB, etc)
  • Concerns

 

Mental Health information:

  • Diagnosis
  • Past, Current and possible treatment(s)
  • Concerns

 

HH conducts inter-agency team meetings to review, discuss and decide on HH’s ability to meet the needs of the individual. Should placement be awarded – an invitation is extended through the Support Coordinator to the individual.

Admission will include a joint meeting to orientate the individual and representatives to the Heritage House programming and much information will be disseminated, gathered, discussed and signed to include:

The collection of certified or original documents of the following:

  • Birth Certificate, SS Card, Medicaid Card, Medicare Card, Other Insurance card, etc.
  • Emergency Medical Release
  • Release of Medical Records to HH (if applicable)
  • Universal Release of Information
  • Authorization to Administer Medication(s)
  • Informed Consent for Psychotropic Medication(s)
  • Permission to Photograph
  • Notifications of Policy
  • Human Rights
  • Confidentiality
  • HIPPA
  • Orientation/Mission Statement
  • Participation in Treatment and Discharge Planning
  • Family Guidelines/House Rules
  • Hours of Operation
  • Building Lay-Out
  • Service Guidelines
  • Emergency Preparedness Plan
  • Grievance Procedure

 

Financial:

  • Living Expense Contribution (LEC)
  • Representative Payee Status for Social Security
  • Day Support and/or Residential Activity Fee(s) (if applicable)

 

Physical Intake Includes:

  • Receipt of Medication (Listing and Count)
  • Receipt of Personal Property (Listing)
  • Client Cash Received
  • Bank Accounts – Disclosed and/or Received
